Salem Psychiatric Health’s patient policies and knowledge-base for answers

The Psychiatric Crisis Center is located at 1118 Oak Street SE, just east of Salem Hospital. The Psychiatric Crisis Center is staffed 24 hours a day every day. If you are going there in person, here is a map of the location

You can request your records by filling out Client Records Request Form or come to our clinic at 821 Saginaw St. South to fill out the records request form in person.

SPA does bill insurance. Please check with your insurance company to find out if your provider is covered by your plan. For more billing and payment questions please contact our billing office 503-315-8646.

Salem Psychiatric Associates is a participating provider for several insurance companies. They include:

  • MODA
  • PacificSource (Non-Medicaid)
  • Providence
  • United Behavioral Health
  • Regence Blue Cross
  • MHN (HealthNet)

Please check with your insurance provider for requirements.  Many plans do need preauthorizations.

Medications will be managed and refilled during your appointments. PLEASE PLAN AHEAD. This will help in the following ways:

  • We can confirm your medications, dosages, check for side effects and send in refills.
  • You will be prescribed enough medication to last until it is necessary to see you again.
  • You will have an opportunity to discuss any medication concerns you have directly with your prescriber.
  • You can request information about your prescribed medications or diagnoses.
  • You can contact the refills inbox in the patient portal.

Make sure you have an appointment with your prescriber before you run out of any medications so they can be refilled during this time.

Schedule II medications are closely monitored. Federal law requires that certain medications need a new, original prescription, written by your prescriber each time the medication is dispensed. It is not possible to have these prescriptions with refills.

If you are requesting refill of a prescription for a schedule II medication, please leave your prescriber a voice mail or message us on the patient portal with the following information:

Your name and phone number
Prescription number
Name of the medication
Whether it is immediate release or timed release
Strength of the tablets or capsules
How many you take per day.
Unless you need to come in for an evaluation, you can expect a call back within 48 hours.

Salem Psychiatric Associates phones are answered 8:30am to 4:30pm. The Administrative Office doors are open from 7:30am – 4:30pm.

Policies

Co-Payment and Deductibles

Co-pays and deductibles are due at the time of your visit. We accept VISA and Mastercard. It is definitely best to stay current, but other arrangements can be made if necessary.

Please check with your insurance if deductible applies for these services and how much you have.

Please note there is a $100 late cancel fee (cancelled in less than 48 business hours) and $100 no show fee for missed appointments.

Statements

Statements are sent monthly unless you have a zero balance. Statements will indicate if your insurance has been billed, and how much has been paid. Most insurance companies take 4 – 8 weeks to process and pay claims.

If you’re without insurance, or choose not to use it, payment in full is expected at the time of service. Special payment plans can be made by talking with our billing department at 503-315-8646 before your appointment.
